We are in-network with the following commercial payers:

• Blue Cross Blue Shield (We are part of the Preferred Provider Panel.)

• PacificSource (Smart Choice, Voyager, Navigator [Coordinated Care Network/CCN], Pathfinder, Legacy Health Plan; does not include Legacy Plus Employee Plan)

• First Choice Health*

* Some insurance carriers share their networks, so check your ID card for additional logos indicating this. For example, although we're out-of-network with Kaiser, we can see some Kaiser PPO patients in-network because their plan utilizes First Choice Health providers.

We can accept the following Medicaid coverage:

• CareOregon

• PacificSource Community Solutions

• Trillium Community Health Plan

• DMAP/Open Card

• ...and we may be able to work with other Portland-area Medicaid plans on a case-by-case basis (e.g. OHSU Health Services).

• We CANNOT work with HealthShare Kaiser Permanente or Providence Health Assurance members. They will only cover treatment by providers within their system, and we are not.

• We cannot accept out-of-state Medicaid plans.

We are out-of-network with all other insurance.

• We can still submit claims to your out-of-network insurance.

• Your out-of-network benefits will apply, and we will balance bill you for amounts not covered.

• Note specifically that we are out-of-network with United, Aetna, and Providence.

• We do not submit or accept network gap exceptions for out-of-network plans.
